Update Your Contact Information with VA

Keep your address, phone number, email and emergency contact current so you don't miss important VA health care and benefit information. 

Here are three ways you can update your information: 

1) Online at VA.gov

• Go to www.va.gov/health-care/manage-health or www.va.gov/change-address

2) In person at Eligibility and Enrollment

• Visit our Eligibility and Enrollment office located in Building 107 on our main campus
• Staff will help you complete the form to update your contact information
• For questions about this, you can reach the staff by calling 928-445-4860 ext. 6235

3) By phone - National Health Eligibility Center

• Call the Health Eligibility Center: 877-222-8387 (toll-free)
• Have your Social Security number and new contact information ready
